The earliest direct evidence came from Cintio and colleagues, who compared the faecal microbiome and the end-products of fermentation in healthy and arthritic dogs. They reported measurable differences in microbial composition and fermentation between the two groups, offering the first support for a microbiome-joint axis in dogs.¹
More recent and larger studies have added important nuance. A 2024 study using pet dogs as a naturally occurring model of osteoarthritis examined 93 animals and found no significant difference in overall microbial diversity between dogs with and without osteoarthritis pain. Instead, the authors pointed to intestinal permeability, reflected in markers such as LPS and LPS-binding protein, as the more likely route by which the gut influences joint pain.² A 2025 study of 175 Beagles reached a similar conclusion: overall diversity was driven more by age than by osteoarthritis, yet specific bacterial groups, including Peptococcus, Peptostreptococcus, Clostridiaceae and Coprobacillus, were enriched in the dogs with osteoarthritis.³

The landmark mechanistic study came from Schott and colleagues in 2018. In obese mice, the prebiotic fibre oligofructose restored a leaner gut microbial profile, dominated by the beneficial species Bifidobacterium pseudolongum, reduced systemic inflammation, and protected the animals from the knee osteoarthritis that obesity would otherwise have driven.⁵ It was the first clear demonstration that changing the gut could change the joint.
The most compelling human evidence is a randomised, double-blind, placebo-controlled trial by Lei and colleagues. Across 537 patients with knee osteoarthritis, six months of a Lactobacillus casei Shirota probiotic significantly improved pain and function scores compared with placebo, and lowered a key marker of systemic inflammation, high-sensitivity C-reactive protein.⁶ A large population study has separately linked the composition of the gut microbiome to joint pain and inflammation in people,⁴ and animal work has shown that an oral Lactobacillus rhamnosus strain slowed osteoarthritis progression by reducing joint pain and inflammation.⁷
The honest caveat is the same one that runs through this whole field. Well-controlled treatment trials in dogs are still limited, so much of the strongest evidence is translational, carried across from species in which the joint-gut relationship behaves similarly.⁸ The biology is shared and the signal is encouraging, but dog-specific proof is still being built.
